Individual requesting access must have parental rights or legal guardianship rights. To sign up for access to your child’s My Sanford Chart record, click here Please note that your child’s chart will be accessed through your My Sanford Chart record. Completing this form will establish a My Sanford Chart record for you and your child.
Request Access to Another Adult's My Sanford Chart Record
To request access to the My Sanford Chart record of an adult whose medical care you help manage, please complete both pages of the Adult Proxy Form and mail to the address listed on the form for processing. In order to process your request, the patient must sign the Adult Proxy Form and provide authorization for release of medical information in My Sanford Chart on the page titled "Adult Proxy Authorization for Release of Medical Information". Please note that the patient's chart will be accessed through your (the proxy's) My Sanford Chart record. Completing this form will establish a My Sanford Chart record for you and for the patient.
