Sanford Health is pleased to offer you member information via a Web-based version of our computer systems called My Sanford Chart.

To request an account, submit a My Sanford Chart Consent Form to your clinic, indicating you have read this Terms & Conditions Statement.

We think it is important for you to know how we handle information we communicate via the Internet. This Terms & Conditions statement outlines our practices and our sensitivity to your right to privacy. We reserve the right to revoke access at any time for any reason.

Response to Electronic Communication - Sanford Health will make its best effort to provide a timely response to electronic inquiries. In some cases, the clinic staff that needs to respond to an electronic inquiry or other communication may not be immediately available so a patient should allow at least two (2) business days for a response. Accordingly, emergency situations requiring immediate attention should not be submitted electronically. My Sanford Chart should only be used to request advice for non-urgent conditions. If you have an urgent need to see a medical provider, please contact your clinic by phone. For a medical emergency, please call 911 immediately.

Furthermore, with respect to any electronic communications sent by the patient, we are only able to respond to such communications based on the information provided by the patient. If there is insufficient information provided, we will be unable to provide accurate and reliable services.

Web Site Links - My Sanford Chart may offer links to related medical Web sites not managed by Sanford Health. These Web site link(s) are for a patient's informational purposes only. Sanford Health does not endorse and has not verified the accuracy of the information in/on these Web sites, and the patient should not rely on any of the information found on the Web sites for purposes of treatment or diagnosis.

Minor Eligibility - An individual must be age 12 or older to request a personal account. Parents or guardians of minor children may request access to the minor child's account by following the terms outlined in the "Proxy Access" section of this Terms and Conditions statement. Access may be revoked when confidential care has been provided, parental rights have been restricted, or when required by law.

Proxy Access - Individuals age 18 or older may request proxy access to another individual's My Sanford Chart account by completing the Proxy Access portion of the My Sanford Chart Consent Form and submitting it to Sanford Health. Proxy access for patients under the age of 18 will only be granted to parties with parental rights or legal guardianship over the patient. Parents or legal guardians may be required to produce documentation demonstrating their relationship to the patient. A My Sanford Chart account will be activated for both the proxy and the patient. If the proxy's legal relationship with the patient changes, the proxy must inform the patient's clinic immediately by calling or sending written notice. Sanford Health reserves the right to revoke proxy access at any time for any reason.

Email Privacy - Patients who are users of My Sanford Chart should be aware that they will be notified via email when there is new medical information to be viewed on My Sanford Chart. This means that any person with access to a patient's email will be able to see this notification. This could include the patient's spouse, employer or anyone else that can access a patient's email account. Although no private medical information will be sent, the notification that new medical information is available by accessing My Sanford Chart may be information that a patient would not want others to know. Thus, patients should take this into account when providing an email address.

Please know that if you send us an email communication, it may be shared with Sanford Health staff that assist the physician in providing medical care.

Security and Confidentiality - We afford the same degree of confidentiality to medical information stored on My Sanford Chart as is given to medical information stored by Sanford Health in any other medium. Sanford Health is committed to protecting the confidentiality of your medical information. We limit Sanford Health employees' access and ability to enter or view information based upon their role in your care. Firewalls, passwords, encryption, and audit trails are further used to safeguard your information. We shall identify the records released and note the time and date of access each time a patient accesses My Sanford Chart. We have taken steps to make all information received from our online visitors as secure as possible against unauthorized access and use.

For other than general information viewing, My Sanford Chart must be accessed with a Transport Layer Security (TLS) modern browser. Our TLS web server uses authentication and offers the highest level of encryption technology commercially available (256-bit AES).

You can tell when you are secure by looking at the location (URL) field. If the URL begins with https:// (instead of http://), the document comes from a secure server. This means your data cannot be read or deciphered by unauthorized individuals. You can tell whether you are truly connected to Sanford Health by viewing the digital certificate. This certificate verifies the connection between the Sanford Health server's public key and the server's identification.

User names and passwords provide two layers of authentication and are stored in an encrypted database that is isolated from the Internet. As a My Sanford Chart user, your role in maintaining the security of your medical information is: 1) Changing your password on a regular basis, and 2) Keeping your login ID and password confidential.

Third Party Services - You may choose to use other services provided by third parties in conjunction with your use of My Sanford Chart. Such services may include, but are not limited to, translation services, location services, and other third-party offerings. Any links to such services are provided for your convenience only. Sanford Health has no control over the contents of these services. Sanford Health does not accept any responsibility for these services or any loss or damage that may arise from your use of these services. Such services may require you to submit your information to a third party, which may result in a copy of your information being used and/or stored by a third party. If you decide to access any such services in conjunction with your use of My Sanford Chart, you do so entirely at your own risk and subject to the terms and conditions of use for such services.

Video Visits - Sanford Video Visits are available for any non-emergent condition for a patient via the patient's own My Sanford Chart account or by a person with valid proxy access to the patient's My Sanford Chart account. You must have proxy access to the patient for which you are submitting a video visit. If a Sanford Video Visit is submitted by a proxy for a patient under the age of 18, the proxy must be the parent or legal guardian for that patient. Be sure you are in the correct medical record when submitting a video visit request. The patient must be present to participate in the video visit. Except as set forth above, you may not use your My Sanford Chart account to seek a Sanford Video Visit for another patient.

Proxy access allows a person to access data in another person's medical record. Typically, parents will use this to access their children's accounts. Adults may also use it to access a spouse, older parent, or relative's record. To request proxy access to a family member, click here.

Computer Requirements - A Sanford Video Visit can be completed on a desktop or laptop computer with a webcam, microphone, and a stable internet connection. Internet Explorer and Google Chrome browsers can be used to complete the video visit. You will be prompted to install the VidyoWeb plugin the first time you start a video visit. A Sanford Video Visit can also be completed on an Android or Apple mobile device or tablet via the My Sanford Chart app. Standard internet data rates may apply. Please contact your internet service and/or mobile device provider to determine applicable rates.

Appropriate Use - By requesting a Sanford Video Visit, you agree to abide by the standards of conduct appropriate for an in-person clinic visit. Explicit, threatening, or abusive behavior or communication or other activities which are not compatible with appropriate use of Sanford Video Visits will not be tolerated and may warrant termination of a Sanford Video Visit.

Privacy Assurance - Your Sanford provider considers your Sanford Video Visit to be a private patient encounter. By accepting these Terms and Conditions of Use, you acknowledge and accept responsibility for any third party in your location who observes your Sanford Video Visit or who accesses your computer or mobile device in any fashion.

Photographs - Please note that depending on your complaint or condition, it may be necessary for your provider to take a photograph during your Sanford Video Visit. By accepting these Terms and Conditions of Use, you consent to such photography if appropriate for your care.
